Even if you filled out the hard copy emergency medical forms, we ask that you still complete the online forms because the online forms will eventually take the place of the paper copies. They can be filled out on a computer at home, at a kiosk computer in any one of our school offices at the High School, Middle School or Elementary School or even on your smart phone. The link can be found on the main Indian Lake Schools website at ils-k12.org under the "Parents" tab. You will need your student's ID number to get registered. We sent home a letter with each student's ID number at the beginning of the school year. Students know this number as their lunch code. If you or your student need that code, please call the School office at 686-8833 (or 686-7323).   If you have already filled out the online forms, you may disregard this message. Thank you!

Indian Lake Schools is trying something new by going to an online forms system. That means you should not have to spend a lot of time filling out those "beginning of the year" forms from now on.
Please click here to see a copy of the letter that is going home with each student. The letter includes your student's ID number, that is required for the online portal. On the back of the letter you will also find detailed instructions that walk you through the online forms process. You will find the portal under the "Parents" tab on the front page of the Indian Lake Schools website at www.ils-k12.org.
